Full Circle Brotherhood — Participant Liability Waiver & Release
This Liability Waiver and Release of Claims ("Waiver") is entered into voluntarily by the participant identified below ("Participant") in connection with participation in the Full Circle Brotherhood basketball program held at the designated activity location in Marshall, NC ("Facility").
1. Description of Activity
Full Circle Brotherhood hosts a weekly basketball session every Tuesday from 7:30–8:30 PM at the designated activity location in Marshall, NC. This activity involves physical exertion, contact sport, and use of the designated gymnasium. Participation is voluntary and open to adult males who have attended the preceding Bible study session.
2. Assumption of Risk
I understand and acknowledge that participation in basketball and physical activities involves inherent risks, including but not limited to: physical injury, sprains, fractures, concussions, or other bodily harm. I voluntarily assume all risks associated with participation, whether known or unknown, foreseen or unforeseen.
3. Release of Liability
In consideration of being permitted to participate, I hereby release, waive, discharge, and covenant not to sue Full Circle Brotherhood, its organizers, volunteers, leaders, and the owners and operators of the designated activity location (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury — including death — that may be sustained by me while participating in this activity or while on the premises of the designated activity location.
4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in this activity, whether caused by the negligence of the Released Parties or otherwise.
5. Medical Authorization
I certify that I am physically fit and have not been advised otherwise by a qualified medical professional. I authorize emergency medical treatment in the event I am incapacitated and unable to make decisions for myself. I understand that Full Circle Brotherhood does not provide medical insurance coverage for participants.
6. Governing Law
This Waiver shall be governed by the laws of the State of North Carolina. If any provision of this Waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
7. Acknowledgment
I have read this Waiver carefully, understand its terms, and sign it freely and voluntarily. I am 18 years of age or older. I understand this is a legally binding document.
